Instructions on Financial Assistance

Financial assistance is provided to poor and needy patients to undergo major surgery etc. out of Governor of West Bengal's Welfare Fund.

Following documents are required to apply for Financial Assistance :-

1. Address Proof (Voter Id Card/ Aadhar Card etc.)
2. Income certificate issued by MLA/Councilor/BDO/Gram Panchayat Pradhan/Concerned Office,
3. Hospital OPD Ticket,
4. Estimated cost of operation/surgery/treatment from the hospital concerned,
5. If possible, recommendation of Speaker of WBLA/MIC/MOS/DM/Leader of Opposition of WBLA.
