Instructions on Financial Assistance

Financial assistance is provided to poor and needy patients to undergo major surgery etc. out of Governor of West Bengal's Welfare Fund.

Following documents are required to apply for Financial Assistance :-

1. Address Proof (Voter Id Card/ Aadhar Card etc.) of the patient. In case of minor, of the parents/legal guardian.
2. One latest passport sized photo of the patient.
3. Income certificate issued by SDO/BDO/Executive Officer (Municipality/Corporation) shall only be considered eligible.
4. Latest Estimated cost of operation/surgery/treatment from the treating hospital with copy of prognosis report.
5. If possible, recommendation of Speaker of WBLA/MLA/MP/MIC/MOS/DM/Leader of Opposition of WBLA.
